1. Payment is due at the time
of service. We accept cash, VISA, MasterCard,
and AMEX.
The cash rate for the initial
visit is $70 and includes taking an initial
history, spinal exam, and treatment.
Follow up visits are $55.
If you are a student or make
less than $25,000/year, please call for
discounted rate info.
2. If you intend to use health
insurance, coverage must be verified BEFORE
you arrive to your appointment. We can check
your coverage for you if time permits, but
ultimately it is your responsibility to
know what your plan covers. If your insurance
is not verified at the time of your appointment,
payment at the cash rate is required. If
your treatment requires additional visits
and it is later determined that you have
some chiropractic coverage, your initial
payment can be applied to your co-pays (deductible,
patient portion) for future visits.
We are in-network for Aetna
PPO/MC NAP and Blue Shield of California.
Other PPO plans may have out-of-network
chiropractic coverage. Since there are many,
we will need to check the coverage beforehand,
and let you know if your plan is one that
we can accept for direct billing.
3. Verification of insurance
is not a guarantee of coverage. What this
means is that your insurance plan may tell
us/you that you have chiropractic coverage,
but other factors (often found out after
the fact) can influence reimbursement to
our office such as deductibles not having
been met, lapse in coverage, non-covered
procedures, and medical necessity. You will
be responsible for any balance left over
after insurance payments are applied to
your account, unless you made other arrangements
with this office prior to your treatment.
4. If you are coming in for
treatment related to a recent car accident,
your primary insurance is your (or the driver
of the car you were in) auto policy’s
Medical Payments (Med Pay) coverage, should
it be available. Most group health plans
will not cover medical treatment for auto
accident-related injuries if Med Pay insurance
is available. Med Pay covers 100% of necessary
medical care, up to your policy limit. If
this applies to you, please provide us with
your auto policy’s information including
the claim number, date of accident, and
claims adjustor’s phone number and
extension so that we can verify coverage.
Please note that if you were not at fault,
using your Med Pay coverage will not cause
your insurance premiums to go up. You can
confirm this yourself by calling the Department
of Insurance help line at (800) 927-4357.
5. If you are coming in for
treatment related to a car accident or other
personal injury and expect a 3rd party insurance
(the at-fault party’s liability insurance)
to cover your treatment, you still are required
to pay at the time of service. 3rd party
insurance typically does not reimburse the
provider directly; only the claimant. Once
you pay, we can provide you with a statement
of your account that you can use to get
reimbursed by the 3rd party insurance.On
some occasions, at the doctor’s discretion,
we may agree to treat you on a personal
injury attorney lien, whereby you don’t
pay until your case settles, and your medical
bills are taken out of your award. This
requires that you get a qualified personal
injury attorney who meets our approval.
6. If your condition is a
Workers Compensation case, pre-authorization
is needed. You will need to file a claim
through your employer; then provide our
office with the claim number, date of injury,
and claims adjustor’s phone number
and extension. Oftentimes authorization
for care may be given on the spot.
